Independent Contractor Agreements

Attached, you find the Independent Contractor Agreement and required documents. All Independent Contractors must have background checks, liability insurance, TB clearance, and a signed contract. This is to ensure the safety of our students. There have been instances where students were mistreated or injured by people who did not have insurance, could not pass background checks, or health clearances. Our students' safety is of our utmost concern. 

Please let us know if you need assistance with obtaining the TB Clearance as we have options. When you obtain fingerprints for the background check, please use the attached form. You will pay a fee (usually between $20 & $30) to the location where you obtain fingerprints. Then we will deduct $49.00 from your first payment to pay the Department of Justice fee.
