Becoming a Vendor with Arete Charter Academy

We are excited about the possibility of working with you for the 2026–27 school year! To ensure the safety and well-being of our students, all vendors/independent contractors must complete the following onboarding requirements.

Required Documentation

  • 2026–27 Independent Contractor Agreement: Must be re-signed each school year.
  • Signed W-9 Form: Required for all new vendors or if your business address has changed.
  • Certificate of Insurance: Please provide a copy of your current liability insurance ($1,000,000 minimum) listing Arete Charter Academy or Pleasant Ridge Union School District as an “Additional Insured.”

Safety & Health Clearances

  • LiveScan Fingerprinting: Any employees teaching lessons in person or online must be fingerprinted. You may indicate that your organization is the Custodian of Records, or employees may use the form attached to our contract.
    Note: There is a small out-of-pocket fee (approx. $25) at the LiveScan location; Arete will then deduct the DOJ processing fee ($32.00–$49.00) from the first invoice.
  • TB Clearance: Per California law, all staff working with students must be cleared of infectious tuberculosis.
    • Option A: Complete the attached TB Risk Assessment form with your own healthcare provider.
    • Option B: If you do not have access to a provider, you may schedule a screening with our District Nurse, Gina Shield, at [email protected].

A Note from our District Nurse

To help our health office run smoothly, please keep the following in mind regarding TB clearances:

  • Email is best: Since she moves between school sites, please contact her via email rather than phone for appointments.
  • Use your provider if possible: To ensure she can assist those without healthcare access, we kindly ask that instructors with their own doctors use the attached form to seek clearance through them first.
  • Arete Instructors only: Please only have instructors specifically working with Arete students reach out for screenings.

CA Mandated Reporting

    • CA SB 848 is a bill aimed at preventing child abuse, including sexual abuse, from happening on school grounds, by school staff or volunteers, or in school-sponsored programs (including Independent Contractors).

  • You will need to create an account and complete the following two courses:
  • (SB 848) Child Abuse Prevention Training for School Personnel and Child Abuse & Neglect Mandated Reporter Training
  • You will have to take two exams to show you understand the information and pay $10.
  • You will receive two different certificates.

Next Steps

Please review the attached 2026–27 Vendor Contract. To begin the process, return the completed contract along with your Insurance Certificate and W-9 to me via email.
