Parents need to bring:

• Proof of Residency (Utility Bill, Lease agreement, car registration, etc)

• Birth Certificate, hospital or county), Passport, Immigration Certificate, or Baptismal Certificate

• Heath Requirements:

  • Record of Immunizations 

  • Oral Health Assessment Form (Signed by Dr.)

  • Report of Health Exam (Signed by Dr.) *TK at start date

There will be three “Stations” - 

• Office Front Desk - Justine will make copies of documents

• MPR Table 1 - Nurse Gina & Ms Sheila will review immunizations, Oral Health, and Health Examination paperwork 

• MPR Table 2 - Ms Andrea will get PowerSchool Forms open on a Chromebook for parents to fill out.
